What is this change?
This PR updates the approval capability and the Postgres write override skill to ensure the original requester is always tagged/mentioned when a requested action is confirmed as complete in a thread.

The principal operator requested this addition in a thread feedback message: "next time also tag the requestor once the action is done! 👍 to inform them".
