Add additional eholdings export fields:

• packageDisplayName;
• managedAlternativeNames;
• customAlternativeNames;
• managedDescription;
• customDescription;
• packageAccess;
• hideInPublicationFinder;
• hideInFullTextFinder;
• excludeFromMARCExport;
• titleProxiedUrl

Remove eholdings package fields:

• packageShowToPatrons
