Add macro-locations.ql and macro-invocations.ql to extract source-tree macro
definitions and call-site locations.
Key changes:
* Add queries/macro-locations.ql to select macro names and location spans.
* Add queries/macro-invocations.ql to select macro invocation names and call-site
spans.
* Filter entities using exists(...getFile().getRelativePath()) to drop compiler
built-ins (<built-in>), command-line defines (<command-line>), and host toolchain
headers outside the source tree.
